Restorative Care Pathway
A short-term, clinically focused program designed to maintain or regain functional ability, reduce or prevent functional decline, and delay higher-level care. It provides intensive allied health and/or nursing services for up to 16 consecutive weeks with funding per episode.
What we offer
Allied Health & Nursing services delivered by a multidisciplinary team
Where available – OT, Physio, Nursing, Dietetics, Speech Pathology, Massage Therapy, AHA
- Goal Plan (& clinical coordination) client goals & priorities, recommended interventions, services and visit frequency, risks/safety considerations, and how outcomes will be measured
- Progress reviews and outcome measurement throughout the episode
- Exit Plan and supported transition to ongoing services where required

Who is eligible?
How to refer
- Send your referral with the timeframe / episode dates and the scope you want delivered. We will provide the requested services within the timeframe you specify.
- We confirm receipt and schedule required appointments within 24 hours.
- You receive the Initial Assessment / Goal Plan report within 24 hours of the appointment, plus the confirmed service schedule.
- At program end, you receive the Exit Plan (when delivered by us) with outcomes and recommendations.

Aged Care Assessment and Support Plan
For Nursing (in VIC) Health Summary and medical diagnosis
Current support/services relevant to the participant's goals
Budget allocation and any discipline/frequenct preferences
If services-only: required services and funding allocations by discipline
Typical timelines from referral
Within 48 hours: We confirm receipt and schedule appointments
5 business days: Initial Assessment completed (Goal Plan referrals) or services commence (services-only referrals)
24 hours of the first appointment: Initial Assessment / Goal Plan report sent to you
Throughout your nominated episode timeframe: We deliver the time-limited program with progress reviews
