Flexible referral pathways: Physio to AHA
Transitioning between services is fluid and driven by clinical appropriateness
Pathway 1: Standard Early Support
Physio assessment and exercise program creation, with AHA support starting after the 3rd session
Pathway 2: The Hybrid Approach
Alternating between Physio and AHA sessions to provide close clinical oversight while maintaining therapy frequency.
Pathway 3: Maintenance & Sustainability
Ideal for existing participants requiring long-term exercise maintenance or those looking for a more cost-effective therapy frequency.
