Our Home Care Occupational Therapy (OT) team help people live safely and independently. They employ a range of strategies to improve functional independence, increase confidence in daily activities and engage in the community.

We work with people aged 15+ addressing a wide range of conditions ensuring we cater to each person’s individual needs.

We aim to enhance daily activities, ensure safety, and optimise the home environment for better functionality. Our OTs utilise an array of equipment, tools, and strategies. With strong partnerships with equipment and home modification suppliers, we ensure your requirements are met seamlessly and hassle free.

Restorative Care Pathway

A short-term, clinically focused program designed to maintain or regain functional ability, reduce or prevent functional decline, and delay higher-level care. It provides intensive allied health and/or nursing services for up to 16 consecutive weeks with funding per episode.

Occupational Therapist (OT) – led Telehealth assessment aligned with Support at Home requirements. Selecting the right personal alarm is essential for participant safety-and under the Support at Home Program, a clinical assessment and prescription from an Allied Health Professional is required before a personal alarm can be funded.
