INITIAL ASSESSMENT (2 HOURS)

The Initial Assessment enables our team to understand the participant’s current clinical needs, goals, and support requirements, ensuring appropriate services can be recommended and coordinated.

Following the assessment, we will:

• Provide recommendations for ongoing care and support
• Arrange any required follow-up services
• Develop a care plan tailored to the participant’s needs

If treatment is required during the assessment visit, such as wound care, this can usually be provided on the day. Please note that additional charges may apply, as treatment services are not included in the standard Initial Assessment fee.

ADDITIONAL ASSESSMENT 

Additional assessments can be requested at the time of referral and completed following the Initial Assessment where a specific clinical need has been identified. Simply let us know when submitting the referral if any of the following assessments are required:

Additional assessments can be scheduled alongside the participant’s usual care visits where appropriate.

TREATMENT ASSESSMENT 

Treatment services can be provided during the assessment visit where arranged in advance. If additional treatment needs are identified during the visit, these can usually be completed at the same appointment where clinically appropriate and will be charged separately from the assessment.

• Wound Care – Assessment and dressing of existing wounds
• Other Clinical Care – Urgent or unplanned clinical interventions identified during the visit, such as stoma care, clinical observations, escalation of care, or other nursing treatments required to address the participant’s immediate needs
