Supporting Nutrition at Home

Good nutrition is essential for health, independence, and quality of life. For participants receiving home care, meal delivery services can be a vital support — but only when they are safe, appropriate, and clinically justified.

That’s why our Dietitians conduct a full Initial Assessment when reviewing an existing meal delivery service or assisting a participant to start a new one.

This thorough process ensures every recommendation is evidence-based, tailored to individual needs, and aligned with participant and carer expectations.

How We Support Participants

When a Care Manager requests a meal delivery service for a participant we haven’t supported before, our Dietitians complete an Initial Assessment that covers:

Liaising & Communication

  • Contacting GPs, Speech Pathologists, or other providers if concerns arise
  • Requesting medical updates or swallowing assessments
  • Communicating with meal providers about participant needs
  • Engaging with Next of Kin or Care Managers

Why Initial Assessments Matter

A thorough initial assessment is essential to understand a participant’s nutritional needs, dietary preferences, medical conditions. Without this, participants may be at increased risk of;

  • Malnutrition, illness, and infection
  • Poor management of conditions (diabetes, bowel dysfunction, cardiovascular disease)
  • Increased risk of falls due to muscle loss and fatigue
  • Decline in independence and quality of life
  • Unsafe or inappropriate meal service recommendations

Ongoing Reviews

Our Dietitians recommend a 12-week review after commencing a meal delivery service, with earlier review for clients at higher nutritional risk or if concerns arise. This ensures the intervention is safe, effective, and aligned with participant goals.

Regular reviews help identify:

  • Inconsistent use of provided meals
  • Taste, texture, or variety concerns
  • Clinical or functional needs not being met
  • Lack of progress toward goals (e.g., energy, weight stability, condition management)
  • By adjusting meal choices, providing education, and liaising with providers, we keep participants on track with their nutritional goals.
