We play an important role in ensuring that people who live at home receive suitable information and/or assistance so your clients can take their medicines as prescribed. Our home nursing team liaise between family GPs and local pharmacists, to deliver safe and effective medicines administration, including IV therapy.
This essential service is particularly important when a person is struggling with memory loss or has Dementia, and is unable to take responsibility for their own medicines, or their carer needs help in managing and administering their loved one’s medicines.
All consumers will be assessed on their ability to manage their medications at the first visit, annually and as care needs change, and in accordance with relevant program guidelines to ensure care needs are met.

We offer a broad range of care services to help meet the challenges of managing diabetes and create a suitable plan and daily routine that fits in with their life. Depending on the type of diabetes a person may have, medication may always be a part of their life and blood sugar monitoring may also need to be monitored on a regular basis. Our home nurses are available to support and manage medications and blood sugar monitoring for those people living with diabetes.

Our Nursing Team complete the assessment using validated tools (PAS or RUDAS) and provide you with the assessment outcome on the same day.
This assessment is completed within a 1 hour visit.

Following discharge from a hospital stay, we will consult with your client’s doctor to personalise the nursing service they need to restore their health and independence at home.
Our caring and respectful nursing team are experienced at delivering care support services for terminally ill patients in their own homes or the home of relatives, or care homes for the frail or aged – patients not in a hospital or in-patient palliative care unit.
We aim to provide comprehensive support for the person, their family and other carers, and support which embraces physical, psychological and spiritual needs.
Multidisciplinary care is available to palliative care patients to assist with control of pain and symptoms, hospital or hospice admission when required, equipment, social support and counselling.
Empower’s nursing team support all ages of people managing catheters. Whether it’s younger people who manage their own care, to adults with complex needs, older people who manage their own care, and frail older people with complex needs, our nurses are skilled and knowledgeable in this area with the prevention and management of common issues arising from catheterisation.

We acknowledge all Home Care & NDIS referrals within 4 hours. Appointments are booked within 48 hours.
Initial appointments completed within 14 days of referral.
The same clinician will be assigned to all appointments for a consumer.
We update Case Managers on the same day or within 24 hours of initial appointment.
There are no hidden fees. Our pricing includes travel, treatment and documentation time.
